Under the current Memorandum of Understanding (MOU), the Honest Broker maintains copies of several public health datasets. These data contain Medicaid information on patients, providers, facilities, and cost codes associated with Medicaid patients.<\/p>\n\t\t<\/div>\n\t\n\t\n\t\n\t\n\t<\/div>\n\t\n\t<\/div>\n\n\n\n<div class=\"col-12 col-md-6 mb-6\">\n\t\t\t\n<div class=\"card   mb-6\">\n\t\t\t\t<div class=\"card-header\">\n\t\t<h3 class=\"card-title\">Medical Electronic Disease Surveillance System (MEDSIS)<\/h3>\n\t<\/div>\n\t\t\t<div class=\"card-body\">\n\t\t\t<p class=\"card-text\">MEDSIS is a statewide system hosted and supported by the Arizona Department of Health Services (ADHS) for use by local health departments for disease surveillance, and for individuals and institutions responsible for reporting communicable diseases. Data includes deidentified and identifiable surveillance on several reportable diseases including COVID-19 as well as the Injury Module which contains reportable overdose incidents.<\/p>\n\t\t<\/div>\n\t\n\t\n\t\t\t\t\t\t\t\t\t\t<div class=\"card-button\">\n\t\t\t\t\t\t\t\t\t\t<a href=\"https:\/\/dataverse.asu.edu\/dataset.xhtml?persistentId=doi:10.48349\/ASU\/9CXRSE\" class=\"btn btn-md btn-maroon\"  >MEDSIS<\/a>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\n\t\n\t<\/div>\n\t\n\t<\/div>\n\n\n\n<div class=\"col-12 col-md-6 mb-6\">\n\t\t\t\n<div class=\"card   \">\n\t\t\t\t<div class=\"card-header\">\n\t\t<h3 class=\"card-title\">Arizona State Immunization Information System (Immunization Data) (ASIIS)<\/h3>\n\t<\/div>\n\t\t\t<div class=\"card-body\">\n\t\t\t<p class=\"card-text\">The Arizona State Immunization Information System (ASIIS) is an immunization registry which contains immunization data on individuals. Arizona state law requires providers to report all immunizations administered to children 18 years of age and younger to the state&#8217;s health department.<\/p>\n\t\t<\/div>\n\t\n\t\n\t\t\t\t\t\t\t\t\t\t<div class=\"card-button\">\n\t\t\t\t\t\t\t\t\t\t<a href=\"https:\/\/dataverse.asu.edu\/dataset.xhtml?persistentId=doi:10.48349\/ASU\/Y4G9C4\" class=\"btn btn-md btn-maroon\"  target=\"_blank\">ADHS Immunization Data<\/a>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\n\t\n\t<\/div>\n\t\n\t<\/div>\n\n\n\n<div class=\"col-12 col-md-6\">\n\t\t\t\n<div class=\"card   \">\n\t\t\t\t<div class=\"card-header\">\n\t\t<h3 class=\"card-title\">Arizona Hospital Syndromic Surveillance<\/h3>\n\t<\/div>\n\t\t\t<div class=\"card-body\">\n\t\t\t<p class=\"card-text\">As part of the National Syndromic Surveillance Program at the Centers for Disease Control and Prevention, ADHS collects hospital syndromic surveillance data from several facilities across the state. Symptoms and diagnosis information or a group of symptoms that consistently occur together (syndromes) are tracked for patients in participating healthcare facilities. These data are collected based on hospital visit information using chief complaints and diagnosis codes.<\/p>\n\t\t<\/div>\n\t\n\t\n\t\n\t\n\t<\/div>\n\t\n\t<\/div>\n\n<\/div>\n\n\t<\/div>\n\n<\/div>\n\n<\/div>\n\n\n<div class=\"wp-bootstrap-blocks-container container mb-0 my-6 my-xl-12\">\n\t\n<h2 id=\"adhs-data-requests\" > <span class=\"highlight-gold\">Requesting ADHS health data<\/span><\/h2>\n\n<div class=\"wp-bootstrap-blocks-row row justify-content-between\">\n\t\n\n<div class=\"col-12 col-md-12\">\n\t\t\t\n<div class=\"wp-bootstrap-blocks-row row\">\n\t\n\n<div class=\"col-12 col-md-7\">\n\t\t\t\n\n<p>The Honest Broker will submit a request packet on your behalf and supply the data upon approval by the Arizona Department of Health Human Subjects Review Board (HSRB). The request packet includes three required HSRB forms plus any relevant IRB documentation. <\/p>\n\n\n\n<p>Before submitting your request, ensure you have completed the Institutional Review Board (IRB) process and your proposed study is approved or exempted. Requests for health data can be submitted by completing the request form. PDFs of all the required documentation can be uploaded via the form. <\/p>\n\n\n\n<p><strong>Please note:<\/strong> Completion of ASU approvals does not guarantee ADHS approval.
